SoonerCare Choice is a primary care case management (PCCM) program in which each member has a medical home that provides basic health care services.
It offers comprehensive care, beginning with a primary care provider who coordinates all of the individual’s medical care. The primary care physician (PCP) serves as the individual’s medical home in which access is facilitated to specialty care, educational services, out-of-home care, family support, and other public and private community services important to the overall health of the patient.
Children and adults younger than 65 with incomes at or below 185 percent of the Federal Poverty Level (FPL) are qualified. While some services for adults require a co-payment, no monthly premiums are associated with this program.
Members enrolled in SoonerCare Choice can change their primary care provider daily over the phone through the SoonerCare helpline (1-800-987-7767).
